It's a way for students, parents, and staff to
1) request support for an IPoly student
2) make an appointment with Student Support Staff
The form responses are CONFIDENTIAL, will ONLY be shared with NECESSARY IPoly staff, and is monitored Mon-Fri (8:30am-4:30pm) excluding holidays and school closures. We will respond to all requests within two days.
